Is Kratom Like Opioids? The Rise of Synthetic Kratom Isolates and Their Risks

Green kratom leaves, powder in a spoon and bowl, and capsules are arranged on a wooden surface.

If you’ve ever used or thought about using Kratom, one question might have popped into your head: “Is Kratom like opioids?”.

It makes sense to ask this question since there’s more and more data coming out about how there are similarities to opioids, as well as side effects and withdrawal symptoms.

Here’s a little spoiler: The answer to our question becomes rather concerning when we’re talking about synthetic Kratom isolates.

What Are Synthetic Kratom Isolates and Why Are They Riskier?

Traditional Kratom (made from the leaves of the Mitragyna speciosa tree) contains 20–40 natural alkaloids. Each one of them has potentially different effects.

In Southeast Asia, it’s been used for centuries as a mild stimulant for workers in the fields, or as a natural pain reliever brewed into tea. This whole-plant form is complex: its multiple compounds often “balance” each other out. That makes the effects more gradual and predictable.

Newer products may contain concentrated or enhanced amounts of 7-OH far beyond the trace levels found naturally in kratom leaf. A separate category includes fully synthetic compounds related to 7-OH. An enhanced extract, an isolated alkaloid and a synthetic compound are not automatically the same product, even when all are marketed using the kratom name.

In nature, 7-hydroxy is barely present in the leaf. But when it’s produced in a lab and sold as a stand-alone ingredient, it acts almost identically to pharmaceutical opioids.

That means the answer to the question “Is Kratom like opioids?” changes from “sometimes” to “almost always” when it comes to isolates. With these concentrated forms, the risk of dependence, dangerous side effects, and even overdose skyrockets.

Many of these synthetics forms of Kratom are sold without quality control, dosing guidelines, or clear labeling. That makes it even worse.

Whole-Plant vs. Isolate

In the natural leaf, the 20–40 alkaloids can balance each other’s effects. Isolating just one can lead to stronger, less predictable (and more dangerous) results. Think of it like the difference between chewing coca leaves and snorting cocaine, or sipping coffee versus taking high doses of pure caffeine powder.

That also means that the natural “safety buffers” are lost with modern synthetic Kratom. Historically, people harvested, prepared, and consumed whole plants. They got feedback from taste, smell, and ritual. With modern isolates, you bypass all that and go straight to a highly concentrated dose. That increases the risk of taking too much, too fast.

Some products marketed alongside kratom are not derived from the plant at all. They may contain laboratory-made compounds designed to resemble or modify the effects of 7-OH. Limited human evidence, uncertain labeling and differences between compounds can make their effects and risks difficult to predict. When combined with other substances like alcohol or sedatives, it can end in even more dangerous side effects.

Is Kratom Like Opioids? How Synthetic Isolates Affect the Brain

One of the main reasons high potency Kratom isolates are so risky is that they target the same mu-opioid receptors in the brain that drugs like heroin, oxycodone, and fentanyl do. These receptors regulate pain, pleasure, sedation…and even breathing.

Kratom products may be used for pain, energy, mood or opioid-withdrawal symptoms, but their effects vary by product and exposure. A concentrated product can deliver far more of a particular alkaloid in a small serving than ordinary leaf or tea. Repeated use can lead to tolerance, dependence and withdrawal. A small dose today may have little effect a week later. That can push you to take more. This escalates the risk of dependence and Kratom withdrawal symptoms (which can include anxiety, muscle aches, insomnia, nausea, and intense cravings).

These isolates, despite being legal in some areas, are chemically closer to opioids than to traditional Kratom. This makes the dangers of Kratom use much more severe.

Why People Turn to Synthetic Kratom Isolates

Many people begin using synthetic Kratom isolates because they believe they’ve found a “natural” and therefore safer alternative to opioids. The marketing often paints these products as herbal remedies…something you can buy at a vape shop, online store, or even a café without a prescription.

Some are trying to manage opioid withdrawal without medical supervision. They’re hoping that Kratom will take the edge off symptoms like body aches, anxiety, insomnia, and low mood.

Others are looking for a “legal high”, for something that delivers the relaxation or euphoria of prescription painkillers or heroin, but without the stigma or legal risk.

The 3 Hidden Dangers of High Potency Kratom

The dangers of Kratom use increase dramatically when we’re talking about synthetic isolates rather than whole-plant powder or tea.

The problem is that when you ask “Is Kratom like opioids?”, the answer moves much closer to “yes” with these high potency Kratom products.

1. Overdose potential

Because these products are so concentrated, it’s incredibly easy to take too much, even for experienced Kratom users. Kratom overdose signs can include:

    • Slow or shallow breathing
    • Extreme drowsiness or inability to stay awake
    • Confusion or disorientation
    • Loss of consciousness

The FDA has warned that concentrated 7-OH products are potent opioid products that may cause serious harm, including respiratory depression. A person who cannot be awakened or is breathing slowly or irregularly needs emergency help. Give naloxone if opioid exposure is possible and naloxone is available, then call 911.

2. Severe withdrawal symptoms

The more potent the Kratom, the more Kratom withdrawal symptoms you may face when quitting. These can include intense muscle aches, anxiety, irritability, nausea, insomnia, and powerful cravings. For some, the withdrawal can feel as bad (or worse) than what they were originally trying to escape.

3. No quality control

Most synthetic Kratom isolates are unregulated. There’s no standardized dosing, no safety testing, and no reliable labeling. One batch might be relatively mild while the next could be dangerously potent. It’s very unpredictable and that is a major factor in accidental overdoses.
